Insurance Participation, Fees & Financial Responsibility

I am currently contracted and in network with the following insurance plans:

Aetna
Anthem Blue Cross Blue Shield
Nevada Medicaid (Fee-for-Service)
UnitedHealthcare
UMR

My standard session rate is $175 per hour.

For commercial insurance plans, clients are responsible for any applicable copayments, coinsurance, or deductibles as determined by their individual insurance plan. These amounts are due at the time of service.

For clients enrolled in Nevada Medicaid Fee-for-Service (FFS), covered behavioral health services are provided in accordance with Nevada Medicaid FFS rules and reimbursement guidelines. Medicaid members are not billed for covered services. Any services that are not covered under Nevada Medicaid FFS will be discussed in advance, and client consent will be obtained prior to scheduling.

For insurance plans not listed above, services are provided on an out-of-network basis. Out-of-network benefits and reimbursement amounts vary by plan. Clients are encouraged to contact their insurance carrier directly to verify coverage, deductibles, and coinsurance.

When applicable, a superbill can be provided upon request to assist with submission of out-of-network claims for potential reimbursement. Reimbursement is determined by the insurance carrier and is not guaranteed.

EMDR INTENSIVE RATES

EMDR Intensives are structured, multi-hour therapy sessions. Insurance coverage typically allows billing for only the first hour of an intensive session. Additional hours are considered self-pay.

The fee for EMDR Intensives is $175 per hour.

GOOD FAITH ESTIMATE

Under the “No Surprises Act” law, health care providers need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided.

You have the right to receive a Good Faith Estimate for the total expected cost of any healthcare items or services upon request or when scheduling such items or services. Once we have completed your initial intake session, I will provide you with a Good Faith Estimate within three days.
