INSURANCE
I operate as an out-of-network provider with all insurance plans. Please consult your insurance plan regarding your out-of-network benefits for potential reimbursement. I am pleased to offer you a "superbill" to facilitate the submission of out-of-network claims to your insurance plan.
EMDR INTENSIVE RATES
90 minute intake session / $260
3 hour intensive session / $630
1 hour follow up session / $175
PAYMENT AND CANCELLATION POLICY
Payment for intake and follow up sessions are due at the time of service. If you are unable to attend an intake or follow up session, please make sure you cancel at least 24 hours beforehand to avoid late cancelation or no-show fees
Intensive sessions require a 50% deposit to schedule. The remaining balance is due at the time of service.
GOOD FAITH ESTIMATE
Under the “No Surprises Act” law, health care providers need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided.
You have the right to receive a Good Faith Estimate for the total expected cost of any healthcare items or services upon request or when scheduling such items or services. Once we have completed your initial intake session, I will provide you with a Good Faith Estimate within three days.
